Not in England. There may be proposals to extend coverage but most
current laws say dishonestly or in order to gain a pecuniary
advantage so just deception isnt usually enough nor even intention to
bypass Choose To Refuse or ACR.

And I posted here about getting calls from 020 2222 2222 and 1111 so BT
doesnt prevent fakes passing over their network nor is there any
obvious way of complaining.

I used to spoof numbers like 020 0000 0000, 0121 000 0000, 0121 987 6543,
011111 and others. They were certainly passed through on the display to BT
customers I called.

No, but some carriers try to validate the number is sensible making it
impossible for us to call from quite correct 03 numbers via them. They
seem not to cope with 020 3 numbers either. It is all very well checking
numbers are sensible as long as you keep up to date!
