How much does it cost to change ISP mid-contract? Ive been with Tiscali
about 20 months now and its
gone from perfectly OK, to shite in about 3 months.

Will I have to pay off the rest of my Tiscali contract?

This is a contract matter between you and Tiscali. You could argue that
they are in breach of contract by not providing you with a satisfactory
standard of service.
